From Usman, the developer: I originally routed every audio request through MP3 regardless of what format the user actually asked for internally, then converted again if they wanted M4A — a double transcode that quietly degraded quality for no reason. Fixing it meant restructuring the pipeline so each target format decodes straight from the source stream rather than always passing through MP3 first. It was a bigger refactor than the bug seemed to warrant, but a "small" extra transcode step is exactly the kind of thing that erodes quality without ever throwing an error.
